Proof of addressUtility bill, bank statement, or official letter dated within the last 3 months at your current Irish address.
Photo ID (if applying by post)Passport or driver's licence. Not required for online applications.
Proof of income โ by income type

Last 3 payslipsMust show gross pay, net pay, and all deductions clearly.
Most recent P60 or Employment Detail SummaryAvailable from your employer or from Revenue MyAccount.
๐ผ If you're self-employed
Most recent audited accounts or tax assessmentShowing your actual net income after business expenses.
Most recent Revenue Notice of AssessmentShowing your declared income for the year.
๐ถ If you're on social welfare
Recent social welfare payment letterA letter from the DSP confirming the payment type and weekly amount.
๐ If you're paying rent or mortgage
Rent: Lease agreement or rent bookShowing the monthly rent amount โ this is deducted from your assessed income.
Mortgage: Recent mortgage statementShowing monthly repayment amount from your lender.
Medical expenses help your caseIf you have significant ongoing medical costs, include receipts or a letter from your GP or consultant. This supports a discretionary medical card application even if you're slightly above the income limit.
